Notify Presbyterian Women’s national offices about changes in PW in the Synod, Presbytery or Congregation leadership with this form. Fill out one form per leader change. You may either mail the form to:

or email it. Please note that if filling out the form for PWP or PWS groups, you do not need to fill out information about the congregation.
